CHRISTMAS HAMPER NOMINATIONS

To nominate someone to receive a Christmas Hamper, please complete this form by Monday 16th November.
To help us with our planning please fill in all of the details.

Please note one hamper will be allocated per form completion!

Hampers will be funded by Bridgecare Limited, through tax-deductible donations given for benevolent purposes.

We appreciate your help in completing the brief details of the hardship circumstances of your nomination, eg financial distress, ill health, family in crisis.

You can select if you would like to personally deliver the hamper or have one of our friendly volunteers make the delivery. Please note we will only be able to deliver hampers to the northside of Brisbane.

If you are personally delivering the hamper to the person you have nominated you are able to collect it on:
Wednesday, 18 Nov from 10am-6pm; or on Thursday 19 Nov & Friday 20 Nov from the church during office hours (9am - 4:30pm).

Please be aware that all hampers must be collected prior to Sunday, 22 November.
